All dogs must have current vaccination records on file with us. This includes annual RABIES, DHLPP, and BORDATELLA. All dogs are also required to be on flea prevention program such as Sentinel, Frontline, Advantage or Revolution. If your dog has a flea problem treatment will be given at owners expense.

We are more than happy to give medication to your dog at no extra charge. All medications must come in the original container. The veterinary clinic provides the name of medication and dosage per day that will allow us to properly care for your pet.

FOOD: We recommend you bring your own food. Sudden changes in a dogs diet will upset their stomach. Food will be measured in proper portions in zip-lock bags and labeled: Name, Sat AM, Sat PM, etc.

Spoiled Rotten Pet Hotel, LLC is experienced in caring for dogs of all ages. Puppies and Geriatric dogs are very welcome.

Cancellation Policy: I understand that Spoiled Rotten Pet Hotel, LLC requires advance notice of cancellations as follows: 24 hours for non-holiday reservations; 72 hours for a holiday reservation. I agree to pay $50 or half the amount of the scheduled visit, whichever is less, if i do not cancel in accordance with their cancellation policy.

Photography: I give Spoiled Rotten Pet Hotel, LLC the right to use photographs of my dog as often as desired. I waive right of approval and release Spoiled Rotten Pet Hotel, LLC from all claims which i may have on account of use of publication of any photographs.

This is a contract between Spoiled Rotten Pet Hotel, LLC (hereinafter call “Kennel”)the pet owner whose signature appears below (hereinafter call “Owner”)

Owner agrees to pay the rate for boarding and all costs and charges for special services requested, including all veterinary costs for the pet during the time said pet is in the care of the Kennel. Owners can drop-off or pick-up your pet anytime during office hours. Boarding fees begin the day you check in your pet. If you pick up your pet before 11am you will not be charged for that day of boarding.

By signing this contract and leaving his/her pet with the Kennel, Owner certifies to the accuracy of all information given about said pet on contract.

Kennel shall exercise reasonable care for the pet delivered to Kennel for boarding. The Owner further agrees to be solely responsible for any and all acts of behavior of said pet while it is in the care of the Kennel.

Owner specifically represents that he/she is the sole owner of the pet, free and clear of all liens and encumbrances.

If pet becomes ill or if the state of the animal’s health otherwise requires professional attention , the Kennel, in its sole discretion, may engage the services of a veterinarian or administer medicine or give other requisite attention to the animal, and the expenses thereof shall be paid by the Owner.

Owner agrees that Spoiled Rotten Pet Hotel is not responsible for any items I bring and leave at Spoiled Rotten Pet Hotel that are lost or destroyed.

I have read and agree with all the rules and regulations.
